The System Sees the Pieces. HUE Starts With Your Whole Life.
Most systems ask which program you fit. HUE asks what is happening in your life, what is connected, and what response actually makes sense.
Built From the Realities Systems Disregarded
Systems have had enough information to know that people do not experience life in separate programs. They built that way anyway.
Housing. Health care. Employment. Education. Mental health. Community support.
We start with the whole life those systems divided.
One Ecosystem. Built Around You.
We bring Pathways, Humanitarians, and coordination together around your life. You can come for one specific purpose or several connected needs. You do not have to use every Pathway, and you do not have to be in crisis.
Those connections should lead to a plan you help shape, clear responsibilities, and follow-through when support involves more than one place.
Individualized Pathways
Your priorities shape your Pathway: what you need, what you want to learn, and what comes next. We build the plan with you and adjust it as your circumstances and goals change.
Start HereCoordination Across Systems
You should not have to be the go-between for every organization involved. We help clarify requirements, connect the people involved, and follow up when a handoff does not lead to help.

Follow-through takes people, tools, and clear responsibilities. We’re building partnerships and systems that help Humanitarians track next steps, address gaps, and stay accountable to Members.

Start With Your Life
Tell us what brought you here, in your own words. You do not need to know which Pathway fits or have everything figured out. We begin with what matters to you and what you want help understanding or changing.
Look at What’s Connected
A missed appointment might be a transportation problem. Trouble at work might be connected to grief. We look beyond the immediate issue so the response accounts for what is actually happening in your life.
Build Your Pathway
Your Pathway takes shape with you. Together, we identify priorities, discuss options, and connect the support that fits your circumstances and goals. The plan can change as your life does.
Make Responsibilities Clear
You should know what happens next, what we’re handling, and how you’ll be involved. We explain the steps, who is responsible, when to expect an update, and how to stay connected, so you are not left guessing or chasing answers.
Coordinate and Follow Through
When another organization needs to be involved, a referral is not the finish line. We help make the connection, follow up on whether it led to support, and work through what got in the way if it did not.
Support Your Goals
Learning something new, growing a business, strengthening relationships, or investing in your health can be the reason you come to HUE. You do not have to be facing a crisis to build a Pathway around what you want for your life.
The Five Pillars Behind the HUE Model
Health is shaped by more than medical care. Where you live, what you earn, what you can learn, the care you can reach, and the relationships around you all matter. These conditions are called the social determinants of health.
We use five Pillars to understand those conditions. Our Pathways put that understanding into action. You do not need to know which Pillar something belongs to before bringing it to us.
Education Access & Quality
Being expected to know something is not the same as having been taught it. This Pillar includes school, practical learning, digital skills, and information that helps you make choices.
Health Care Access & Quality
Getting care involves more than finding a provider. Cost, transportation, scheduling, trust, and how you are treated can determine whether care is actually within reach—and whether you return.
Economic Stability
Having a job does not automatically mean having enough to live on. Income, reliable work, benefits, expenses, and opportunities to grow shape your choices and ability to plan ahead.
Social & Community Context
Relationships, culture, identity, faith, and belonging shape daily life. We consider connection, discrimination, rejection, isolation, damaged trust, and what being supported means to you.
Neighborhood & Built Environment
Your address affects more than where mail arrives. Housing, transportation, safety, food access, and the condition of the spaces around you influence what you can reach and how you live.
Pathways for Real Life
The five Pillars help us understand the conditions around you. Our Pathways turn that understanding into support you can use. Start with what matters to you; your Pathway can connect more than one area.

For Members
You can come with one concern, several connected needs, or something you want to learn or build. You do not have to sort your life into our categories first.
You help shape the plan. A Humanitarian works with you to understand your options, make responsibilities clear, and follow through on our part. When your priorities change, the conversation and the plan can change with you.
Start HereFor Organizations
A referral should lead to a connection, not another person left to chase an answer. We work with hospitals, health plans, schools, faith communities, employers, and community organizations to make the next step clearer.
We work out what support is needed, what each organization is responsible for, and how follow-up will happen. Members’ priorities, consent, and everyday circumstances stay part of that work.
